Dear Valued Customer,

Thank you for registering to participate in the voluntary recall of certain models of Jardine cribs on which wooden slats may break. Jardine has worked cooperatively with the U.S. Consumer Product Safety Commission to conduct this voluntary recall.

Enclosed is your recall kit, which includes an acknowledgment form, instructions for removing crib hardware and identification label, an Allen key for removing the hardware and a prepaid return envelope.

Please take the following steps to ensure that no one else uses the crib and to receive a voucher good toward the purchase of another crib at Babies"R"Us or Toys"R"Us stores.

1. Review and sign the enclosed acknowledgment form.

2. Remove mattress support hardware and identification label using the enclosed instructions and Allen key that is provided.

3. Place bolts, mattress support plates, identification labels (or photos of labels) and signed acknowledgment form in the prepaid shipping envelope provided.

4. Send the envelope to Jardine via Federal Express. You may take this to any Federal Express outlet or drop box or Kinkos.

5. About 1-2 weeks after you return the envelope with the crib hardware, identification label and signed acknowledgement form, you will receive a voucher good toward the purchase of any other crib at Babies"R"Us or Toys"R"Us stores, in an amount equal to the highest retail price charged for your crib, including any applicable sales tax.

6. Within 6 months of the date that the voucher was issued, go to a Babies"R"Us or Toys"R"Us store to redeem the voucher for a new crib. If the new crib you choose costs less than the voucher amount, you will not receive the difference. If the new crib costs more than the voucher amount, you are responsible for any additional amount.
